PRIVACY POLICY
This privacy policy governs your use of the software application CareScriptions™ (“Application”), created by Seremedi, Inc., a Delaware company, which includes a mobile application, a web-based component and various cloud services. The Application is an Interactive Remote Health Management solution for patients with chronic conditions, providing assistance in management of treatment plans.
Consent to Privacy Policy
The term ‘you’ or ‘user’ as used in this Privacy Policy refers to a patient, health care professional and any authorized caregiver using CareScriptions™.
The Application allows for you to authorize, validate and invite additional caregivers to use the application on your behalf and access your information via the Application. You acknowledge that Seremedi, Inc. is not responsible for caregivers to whom you choose to grant access.
This Privacy Policy may be updated from time to time as deemed necessary. The most current version, history and changes to the Privacy Policy will be posted on www.sermedi.com, or you may be notified via an email, text message or notification within the Application itself. You are advised to consult this Privacy Policy regularly for any changes, as continued use is deemed approval of all changes.
By using the Application, you are consenting to our processing of your information as set forth in this Privacy Policy now and as amended over time. "Processing,” means using cookies on a computer/hand held device or using or touching information in any way, including, but not limited to, collecting, storing, deleting, using, combining and disclosing information, all of which activities will take place in the United States. If you reside outside the United States your information will be transferred, processed and stored there under United States privacy standards.
User Provided Information
The Application obtains user information when the Application is downloaded and either you or a Health Provider registers you as a user.
Registering with CareScriptions™ and using the Application includes obtaining the following information:
(a) your name, email address, age, password and other registration information;
(b) information related to your care plan and discharge instructions recorded in your Electronic Health Record and provided by your Health Provider;
(c) information you enter into the system when using the Application, such as text messages, recording of vital signs, symptom reports, treatment completion and other data.
Information gathered during Application use is intended to be used by the Health Provider to compare treatment plan effectiveness and adjust individual patient treatment schedules accordingly.
On conclusion of treatment and Application usage, we may De-Identify Your PHI and Your Personal Information, and use and disclose the de-Identified Information as allowed by law.
We may also use the information you provided us to contact your from time to time to provide you with important information, required notices and marketing promotions.
Automatically Collected Information
In addition, the Application may collect certain information automatically, including, but not limited to, the type of mobile device used, mobile devices’ unique device ID, the IP address of the mobile device, mobile operating system, the type of mobile Internet browsers used, location, location-based information and information about the way the Application is used. This information will be used to improve application ease-of-use, environments/technologies supported and enhancing functionality of the Application.
Third party Apps, Services and Technology
We may disclose User Provided and Automatically Collected Information:
- as required by law, such as to comply with a subpoena, or similar legal process;
- when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request;
- with our trusted services providers who work on our behalf, do not have an independent use of the information we disclose to them, and have agreed to adhere to the rules set forth in this privacy statement.
- if Seremedi, Inc. is involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified via email and/or a prominent notice on our Web site of any change in ownership or uses of this information, as well as any choices you may have regarding this information.
This Privacy Policy and security practices are designed to comply with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). As such we maintain protected health information (PHI) in compliance with these rules and our contractual obligations with the health care providers that are “Covered Entities” under the federal health care privacy and security rules (HIPAA and HITECH).
Security
In order to safeguarding the confidentiality of your information, the Application provides electronic and procedural safeguards to protect information processed and maintained. For example, access to this information will be limited to authorized employees and contractors who need to know that information in order to operate, develop or improve the Application. Please be aware that no security system can prevent all potential security breaches.
SMS Messages, Notifications and Content
Electronic communication has a number of risks that include, but are not limited to being copied, forwarded, and stored in numerous paper and electronic files. Seremedi will use reasonable means to maintain security but will not be held responsible for the content, timeliness or any interpretation of these messages.
Standard text messaging rates or other carrier charges may apply when using the Application to receive or send messages and notifications.
By accessing the application you consent to receive communications, and for content and communication you submit, you acknowledge that you have all the necessary rights, licenses, consents and approvals to submit such content, and it does not violate any privacy, public, copy, trademark or any other rights of any person or entity.
